This study outlines an unusual and innovative approach to an issue that will concern a large number of us at some time in our lives, either as professionals or consumers – or both. It is aimed primarily at professionals involved in planning health care strategies, but it could be applied to other areas of care. In looking at a section of the community who (while increasing in numbers every year) remain the Cinderellas of the service, it is brave and creative.

It is good to see academic research turned into a usable guide. I hope that this book will reach many professionals – and students – and that the wider implications of the approach it outlines, and the philosophy which underpins, it will speak directly to health care professionals at every level.
